File: not received two wheeler policy from last one tear

Dear Sir/Madam,
I m Sanjay Kumar from Jaipur. I applied for two wheeler insurance policy to Reliance General Insurance Agent at Vishal Mega Mart M.I. Road, Jaipur office in July 2009 and deposited Rs. 1015/-. Cover Note no. is 209000077545 But till date my money is not deposited by the Agent and have not received my policy also.
