    Question Application for transfer of family pension to spouse

    I want to know the following:
    1. Is there any prescribed form for submitting specimen signature while requesting to transfer the family pension to spouse in a nationalized bank?
    2. is it necessary to provide specimen signature on the prescribed form with attestation by two gazetted officers?
