There is no prescribed form for making application under
RTI Act. You can make it in an ordinary letter form. Indicate that you are making this application under
RTI Act. Specify what details u want clearly. Enclose a Postal Order(PO) for Rs.10/- as fee and mention this in your application. Leave the drawn in favour of column in the PO blank and ask the
PIO to fill it up appropriately. Address the application to the
PIO mentioned in the post of Mr. Karira above.
