Cancellation of lost of passport and getting new one in changed name

My family member was having a passport having her husband name after her and the same is lost. Now she is divorcee. Need has arisen to cancell the said passport and in the new name i.e. her father name & surname after her name. What porcedure is to be adopted to avoid misuse of the lost passport, and get new one in the least time span?
