Need Help: How & where to file an RTI seeking information of the supply of Tender Item for Ram Manohar Lohia Hopital, Delhi

I want an information from Ram Manohar Lohia Hospital, Delhi( comes under Ministry of Health & Family Welfare), regarding the supplies of an order of a surgical Item. I have come to know that it is a routine in the hospital supplies that The Distributors quote the prices of Branded Products & when they are awarded the tender they supply low quality fake products & nobody at the instiution checks the same. I have one such order number with me where a distributor has supplied the goods of local make (the distributor got the order on behalf of a reputed company's authority letter) & the company doesn't even know that any order was awarded to the Distributor.
Please help with the details:
1. Where to file the RTI
2. Process & Fees
3. Application format (if any)
4. Framing of application (so that I can get a proper response)
