PIO of PF Mumbai office as per attached format. Please visit epfindia.com site for exact address. You have to send Indian Postal Order in favour of 'CPFC of EPFO', Mumbai, payable at mumbai for Rs.10/- with the application. All the best. Pl send by speed ad or regd ad post.
