Hello Dr. Anil,
The format for application under RTI has been specified in the Rules framed under the Act by some State. You can go through the rules of your state for such format, if you intend to obtain information from a Public Authority under the State Government. However, these State rules will not apply for obtaining information from a PA under the control of Central Government. Moreover, the PA is under obligation to render assistance to the person making the request orally to reduce the same in writing (S6(1).

(iii) The information requested is not exempted under section 8 and 9 of RTI Act.
(iv) Information requested is connected with corruption and against human rights violation. So there is no exemption under section 8.
(v) The information requested is connected with life and liberty of a person. So the information should be given within 48 hours.
(vi) Send the information through registered post
(vii) Intimate the designation and address of FAA
