How to fill PF form?

Hi Guys,
I have left previous company on 10th March 09,and now i want to withdraw the PF amount.I have joined my new company on 1 june 09.
I have asked my previous company to provide me the forms and they have given me the forms.

1: PF form which contain 4 pages in which one page is of FORM 19.
i am able to fill that particular page only, and rest 3 i dont knw what i have to fill.
One page contains "Advance STAMPED RECEIPT" which contains 3 sections:
a) advance stamped receipt
b) For the use of commissioner's office (i think we need not fill this)
c) for use in cash section.

please guide me what i have to fill in (a) and (c).

second page also contains 3 section :

a) a table which contains months,amount of wages,worker's share like column.
b) Form no.5
C) form no.10

please advice me what i have to fill in (a),(b),(c).

I have attached both the pages so that you can guide me better.
please have a look.
