Is there any prescribed form to seek info from different ministries. In case of DOPT, from where this form can be downloaded.

There is no prescribed form for application under RTI Act 2005. Application can be filed on a plain paper, either typed or hand written. The application should contain the name and address of the applicant, besides other contact details if possible. The applicant may also phrase his/her question properly for the kind of information desired. The name of the office, from which the information is desired may also be mentioned in the application.

This application can be addressed to the SPIO /CPIO of any Govt. Deptt. or Public Body. The concerned PIO will either reply this application or forward it to the concerned Public Body.

Of course, application fee of Rs.10/- or what ever applicable has to be submitted with each application, unless excempted.
