Addition of family name in my daughter's birth certificate
We registered our daughter's name but didnot put the family name when we applied for the birth certificate in Mumbai.
Now we want to include the family name after her given name.

As per Registration of Births and Deaths Act one can get the name modified upto the age of 15 years of the child. You may file an application with the Registrar of Births and Death of your area where the birth of your daughter is registered and request for necessary change. Also apply for the copies of the modified Birth Certificate by depositing requisit fee. In case action is not forthcomming in reasonable time frame, file a RTI application. Thats It.Sponsorer
