Address Change in Driving License

My driving license has a Delhi address whereas I am currently residing in Bangalore. How can I change my address in the driving license to my local Bangalore address ? Doubts which I have regarding this are -

1. Not clear about the exact procedure
2. To change the address what address proof do I need to furnish (of the current address)
3. How much does it cost to do this ?
4. How much time will it take to get this whole thing done ?

Could anyone please help ?
