Dear learned friends i have prepared this application with the help of Karira Sir. Very soon i am going to file but before that i wanted that all my seniors may take a look of that and suggest me so i could be able to reword in right manner.

1) Kindly provide the following information related to prepaid electric meters:
a)Please provide attested copy of the letter written by the electricity dept. in order to introduce prepaid electric meters in GKP region.
b)Were the engineers from GKP sent for training to learn operation & installation of the said meter?
c)How many engineers were sent for this training and what was the duration of this training and also the location of the training center? Kindly provide the complete description of the expenditure done for this training (attested copies of bills and please include salary of those engineers in this too).
d)Please provide attested copy of the letter by which any order was placed before any company to purchase prepaid meter & the attested copy of any quotation/s invited before this order.
e)As this scheme was not implemented, what were the reasons behind this? Who is responsible for all this expenses (gone in vain) done in this reference?
f)What action has been taken against the concerned officer? Kindly provide the attested copy of that letter.
2) Electricity meters that are used by the department in the consumers place to calculate the consumed units, quality and accuracy of that meter has been checked & certified by which agency of the country (B.I.S., ISO, ie.). Please provide attested copy of that certificate
3) Kindly provide the following information related to employees of the electricity department who reside in urban area in multi-story buildings (paternal, maternal, on wife’s name, received in gift, got in donation)?
a)How much money do they pay as average monthly bill?
b) Fixed charges are applicable on them or not
c) Bill details with fixed charges & without fixed charges.
4) Kindly mentions that how many Check meters were installed in the home/s (multi-story) or commercial establishments of the employees of the electricity department (working & retired both in past two years). Any sort of irregularity has been found or not.
5) Under the law whether it is permissible or not. if it has not been installed, then why it is so?
6) Kindly provide the following information related to heightening of electric poles:
a) Some poles heightened up to carry high-tension lines in some areas of the district. To carry the said high-tension line was the only task that was fulfilled by this or any other please provide information.
b) Kindly provide the attested copy of the proposal letter through which this task has been started
c) Height of old pole, height of new pole & difference.
d) Department has done this work by own or tender was called for the same if yes, please provide the attested copy of tender notice (as published in paper)
e) Do all poles meet the above standards?
7)If this task was done by any agency/Contractor then provide details:
a)Work order no.
b)Name of the agency/ Contractor.
c)Actual date of start.
d)Actual date of completion.
e)Status of work.
f)Amount paid or payable.
g)Kindly provide the attested copy of inspection report (inspection to check the quality of the work & material)
h)Provide the names of the officials who inspected and passed the payments.
9.Please provide copy of the letter through which the concerned officer issued orders to reduce the time limit of bill of domestic consumers from two months to one month and also provide the copy of letter written to attain approval of the government (if any). What were the reasons behind such change and also the reasons to withdraw this?
10. Kindly provide following information related to Manyavar Kansiram Samagra Sahari Vikas Yojna:
a) How many wards of the district were selected under this scheme.
b) What task to be carried on in these wards
c) What percentage of such task has been completed, kindly mention detailed description acc. to wards.
11.What is the percentage of Transmission & distribution loss in GKP region? Among them how much loss is by the illegal electricity connections and how much by the other mediums and what are these other mediums?
12.Very soon Ariel bunch Conductor(laminated/insulated wires) are going to be introduced in GKP region. Please provide following information in this regard:
a) What is the difference between present & new (laminated/insulated) wires?
b) Kindly provide the attested copy of letter by which department tried to contact the concerned officers of N.C.R. & Western U.P. regions in order to know how helpful these wires are in order to stop distribution loss (As we know such wires are being used since long time in National Capital Region and Western U.P. Region). 13. Is there any inspection agency for keeping an eye on Transmission & Distribution Losses in Gorakhpur?If yes, what is the name of the inspection agency?
15. How many staff members of Purvanchal Vidyut Vitaran Nigam Limited are trapped by the A. C. B. Unit during 2000 - 2008? Their names.
16. How many staff members are acquitted from the corruption cases by the A. C. B. Unit, Hon'ble Court of Law and Purvanchal Vidyut Vitaran Nigam Limited/Dept.?
17. How many staff members have been convicted by the Court of Law?
18. How many convicted staff members are working with Purvanchal Vidyut Vitaran Nigam Limited and why are they working with Purvanchal Vidyut Vitaran Nigam Limited ? How many convicted staff members are terminated?
19) Whether any appeals or revisions are being filed by the convicted staff members or the ones terminated from Purvanchal Vidyut Vitaran Nigam Limited. Ltd.?
20) Kindly provide details regarding billing work: after completion of tenure of K.L.G., how the new agency was selected for this work. Please provide full details:
a) Date of tender notification.
b) Copy of advertisement in paper.
c) How many agencies have bided for the name. Their names
d) What were the criterions for shot listing?
e) How many were short-listed? Their names
f) On what basis Sterling Transformer was selected for this work?
g) How many years of experience does this agency have?
h) In which areas of India, company has earned that experience. Name of those areas
With reference to the above it has to be mentioned that the information sought for is very urgent and in pursuance of general public interest. Along with the letter please find postal order of Rs.10/ no.____________ dated: 26/07/2008 enclosed as per provisions under the act. Kindly provide the attested photocopies of the documents and inform me of the costs of photocopies within three days of the receipt of the letter so that could be deposited as per law. Any delay on your part would mean your lack of concern towards the constitutional provisions and would force me to follow other provisions provided under the act. Kindly acknowledge the receipt of this
