It is better to stick to the format provided in the relevant RTI Rules.
Just refile the application in the proper format.

Sometimes PIOs will accept applications which are not in that specific format, as long as all the information in the format is given ion the plain paper also.

If all details in your application are the same as those required by the format, then you can certainly make a first appeal for malafide denial of information.

Rule 3(1) of Haryana RTI Rules 2009 states as follows :
A person who desires to obtain any information admissible under the Act, shall make an application, preferably in Model From 'A' to the State Public Information Officer and in his absence to State Assistant Public Information Officer along with fee as specified in sub-rule (1) of rule 5 of these rules.
Form 3A published by the department is only a model form and need not necessarily be used.

File a first appeal as PIO is intentionally denying the information using malafide means.

Copy or Haryana RTI rule 2009
