I humbly request all of you to suggest other questions as you may deem fit and also suggest rectifications, if any, for the questions I have laid down:
Hon’ble Sir,
With reference to the aforementioned subject, information is sought from your office in the general public interest on the following:
1) Kindly provide following information related to Medical/Drug Stores:
a) How many Medical stores are there in Gorakhpur district (urban & rural)? Kindly provide the list of them.
b) How many applications for new licenses of Medical Stores (retail & wholesale) are pending?
c) How many new licenses of Medical Stores (retail & wholesale) issued in last ten years?
d) How many licenses (of medical stores retail & whole sale both) can be issued on the name of one Pharmacist?
e) Please provide the certified copy of G.O. in this regard (with respect to point d)
f) What is the ratio of retail & wholesale medical stores in Gorakhpur (urban & rural)?
g) While issuing licenses does department considers this ratio (ratio of wholesale & retail medical shop)?
h) How one can get drug license? Please provide the details.
i) Kindly inform me the Name, Designation, Office Address, Office Telephone number of the officer/s in your office, who is responsible for the inspection of these medical shops for discrepancies (many medical shops retail & wholesale registered on the name of one Pharmacist, Sale of expiry drugs, sale of sample, sale of govt. supply medicine from medical shops i.e.
j) How often he/she visits these Medical stores (retail &wholesale) for surprise checking and with what frequency?
k) Please provide me the certified copy of the inspection reports made by the officer after these inspections (in last five years). Details of discrepancies found in the inspection.
2) Kindly provide information related National Filaria Control Program:
a) What are the recurrent anti-larval and anti-parasitic measures?
b) By these measures, how much control is being possible?
c) With what frequency the spray of anti-larval solution is done and in which areas?
d) How many liters of this solution (anti-larval) is required for spraying (in a month)?
e) Whom does the spraying of this anti-larval solution benefit?
f) How department procures this solution (anti-larval)?
g) Kindly inform me the Name, Designation, Office Address, Office Telephone number of the officer/s in your office, who is responsible to make arrangements for regular spraying.
h) What are arrangements made by department to ensure regular spray of anti-larval solution and how department monitors it?
3) Kindly provide information related to Pathologies, Diagnostic centers, Ultrasound centers & Collection centers of multi specialty labs:
a) How many pathologies and collection centers of multi specialty labs are working in Gorakhpur dist. (urban & Rural)
b) How many of them are registered?
c) How many ultrasound centers are there in Gorakhpur (urban & rural) and How many of them are registered?
d) Details of staff working in these centers (Pathologies, Diagnostic centers, Ultrasound centers & Collection centers of multi specialty labs) name, address and attested copies of their certificates
Need some questions on RNTCP (revised national tuberculosis control program) & food license

hese are certain queries I made with our local Drugs inspectorSponsorer
hope it may help u a little bit
1. Please provide us the list of all medical stores in the following format.
Name & location of the pharmacy/ Name of the Proprietor/License No./Name & Father’s name of the Pharmacist/ Reg. No./ Temporary & Permanent Address of the pharmacist/ College & year of passing/ Since how long he is working with this particular pharmacy/ Name of the pharmacy he worked earlier/working hours of pharmacy ?
2. How many visits are scheduled in a year by the Drug Inspector to each and every pharmacy?
3. When is the next scheduled visit of the Drug Inspector to each and every pharmacy?
4. What are the books/records to be verified by the drug inspector during his visits ?
5. Can the Owner of the Pharmacy run the store and sell the drugs/medicines in the absence of the Pharmacist ?
6. Can the Owner of the Pharmacy issue Cash bills under his signature in the absence of the Pharmacist ? What if he issues a Cash bill under his signature ?
7. During the next scheduled visit of the drug inspector please make the pharmacist to fill up the following information (in the format given below) as a proof of presence of the pharmacist at medical store ?
Name of the Pharmacy/ License No./ Pharmacist Name/Reg No of pharmacist / Signature of the pharmacist?
(please provide the information after the completion of the next scheduled visits of the Drug inspector to the pharmacies)
8. Can a Pharmacist work for two Pharmacies at a time ? What if he is found working ?

Under National Filaria Control Program:
1.regular spraying of anti-larval solution is done
2.solution is effective for one week only
3.Govt. has sanctioned fund for this
4. They show the spraying on paper
5. money goes in their pockets
RNTCP:
same scene
1) supervisors are hired to look after patient of TB
2) their job is to see that patient has taken medicine timely or not and to make sure that patient must take the medicines timely & regularly
3) good salary & conveyance allowances are paid to him/her
