Applied for passport in 2005 i.e. Three years back and not received till date

Dear Sir,

This is to inform you that I had applied for passport vide receipt no. H00577606 in 2005 and till date have not received the same. I would request you to kindly intervene into the matter and help me out as I have to go to canada to attend conference to be held in September.
