Need Help: Please check application format

I would appreciate if somebody could review the following application format regarding delay in receiving ration card:

Dear Sir,
Please provide the following information under Right to Information Act 2005.
I filed an application [ Form No: xxxxx] for a ration card on 1st March 2008. The residence address verification happened on 19th March 2008.
Six months have passed and I am yet to receive my ration card.
Therefore please answer the following questions in English:

• What is the day by day progress on my ration card application?
• What are the names of the officers-in-charge of processing my ration card?
  • How long has the application remained with them?
  • What action did they take regarding my application during this period of six months?
  • What action will be taken against those officers responsible for this huge delay in the processing of my application form?
• Finally, by when will I receive my ration card?

Dear Sir,
I filed an application [ Form No: xxxxx] for a ration card on 1st March 2008. The residence address verification happened on 19th March 2008.
Six months have passed and I am yet to receive my ration card.

I Mr. XXXXX, a citizen of India would like to request you for the following information under RTI Act 2005.

* Kindly provide me with a copy of my application along with file notings or progress report if any.
* Kindly provide me with a list of all officials who have been in possession of my file since the date of its filing at your office till the date of me making this RTI Application. Such list should contain 1)name of the officer, 2)date when the file was forwarded to the officer, 3)date the file was received by the officer, 4)purpose/reason of forwarding the file to the officer, 5)the date on which action was taken on the file by the said officer.
* What are the set protocols/policies/guidelines under which applications for ration card are processed. Kindly provide a copy of the same.
* What are the provisions under any law, which exist for awarding punishment to an officer for non-performance of allotted duties.
* To whom can such a complaint be made by an aggrieved citizen. Kindly provide details of the authority such as address and any prescribed format that might exist for making the complaint.
* The reason for the delay in processing of my application, and the date by which I may collect my Ration Card from your office.

I declare that the above requested information pertains to your Public Authority to the best of my knowledge. In case it is thought that the information might relate to another Public Authority, then kindly forward such part of my application to the PIO of the other Public Authority under provisions of section 6(3). I would also like to make it known that the applicant is limited to reading and writing in English only, and thus it is requested that all correspondence be made in English itself.

Plz see the questions given below and include them in your RTI application as per your requirement;
1. Daily progress made on my application till date.
2. Names and designations of the officials with whom my application was lying during this period. Please intimate the periods when it was lying with which officer and what action was taken by that official during that period on what dates.
3. Please give a the proof of receipt and dispatch of my application in the offices of each of these officials.
4. According to your rules or citizens charter or any other order, in how many days should such a matter be dealt with and resolved. Please provide a copy of these rules.
5. The above officials have not adhered to the time limit mentioned in these rules and are guilty of violating these rules and hence guilty of misconduct under their conduct rules. Please give a copy of their conduct rule, which they have violated by violating the above mentioned rule.
6. These officials have caused serious mental injury to me by making me run around all this while. Are these officials guilty of causing mental harassment to the public?
7. What action will be taken against these officials for violating all the above rules and for causing mental agony to the public?
8. By when this action would be taken?
9. When will I get my Ration Card?
must include this in your application:I am willing to pay any further charges as decided by the PIO and communicated to me with a reasoned written order, giving detailed calculations for arriving at the additional charges so requested by the PIO. I am enclosing herewith a B/C or D/D number…..issued by…….bank, dated……….for Rs………towards application Fees for this RTI Application.

A smart PIO would have simply rejected the application filed by you, because questions are not entertained under RTi. You can only seek copies of documents held by a public authority.

but your thread wasnt existent at the time when I was making mine. In my post (post#5) i was actually trying to converse with mr vjorohm. Sorry if my post offended you, but it wasnt meant for you. Sorry again!
