Complements for the commendable work. In my experience, the implementation of the act suffers the most because of refusal by public authorities to implement section 25, that deals with monitoring and reporting, properly. This way, PA's close their eyes to the violations of their PIO's and indirectly give them sanction to continue the violations, without any risk of the violations getting quantified and reported. So addition of a few questions in yoor above questionnaire, to expose this intentional misdeed of certain PA's, will make it more effective.
