Please make following changes in format:
Address it to Branch Head/ACPIO SBI, _______ branch.
Amend clause on payment of filing fee as " Cash of Rs.10/- is paid herewith"
Mention dates of all letters and enclose photocopies thereof with RTI application.
In addition you can scale up your complaint to Local Head Office Delhi by visiting website of the bank for address.

No need to approach IT Dept as it is not at fault. It would have credited to some general account meant for errors of TDS deductors etc. There is error and deficiency in service by SBI. Ombudsman should be last resort.
