RTI to know about development activities carried out in a ward

Hello,

I want to frame an RTI to the municipal commissioner of a city to know about the development activities carried out in a particular ward in the last 3 years. Can somebody provide me with a template for the same?

Just mention as under:

Please provide me year-wise information of money expended by the corporation on ward No. __ for last three years ending 31-03-2011, indicating details of each work and amount expended for that work.
