    How to put RTI to employer

    I worked as employee in govt. PSU and resigned. I want to get Service book copy, Leave credit at regular intervals( on 1st jan, 1st july) during my service, Copy of my leave applications. ( Just to compare whether my leaves are deducted as per my applications are not), I want to know reasons for not settling my account(i.e release of resignation benefits, Arrears, leave encashment)


    My doubt is i need ask as citizen of india.
    In applicant details i need to give my name, designation, employee no. and ask queries

    please provide me copy of my service book entries
    Please provide me, photocopy of my leave application forms during 01/06/2005 to 01/06/2011
    Is it correct?

    or

    In from address i mention just my name and address

    Q: please provide me, copy of service book entries of Ramesh, Emp No: 123
    Please provide me, copy of leave applications of sri. Ramesh during 01/06/2005 to 01/06/2011


    which is correct?

    If both are wrong, please guide me correct method
