I applied for the details of open "main hols" n the city from public health. In which I asked them to provide information with in 48 hours being question of life and liberty. I have assured that all the main holes with in their jurisdiction will be covered till monday. i got this information with in one of the filing the application.

Sir,

Being a citizen of India , I am writing to make an open government request for all the information to which I am entitled under RTI ACT 2005. In order to assist you with request I am outlining my query as specifically as possible if however this request is to wide too unclear I would be grateful if you could contact as I understand that under act , you are required to advise and assist requester.

1. Number of Main Holes in the city on road side or in localities .Provide area wise detail.
2. Present and latest status report about the maintenance of these main holes.
3. Provide the data relating to Number of Main holes opens, broken, damaged in the city on the date of making of this application kindly supply report Since how long these are not maintained and why.
4. Supply Relevant rules regarding maintenance of main holes.
5. Kindly supply departmental rules if maintenance of main hole are neglected how they can cause loss, As per departmental rules it dangerous to life if small child or some body else falls in it.
6. Supply the copy of action taken during last 2 years against any official if at any point of time he is found negligent for his duties.
7. What are duties of the head of the department of the Distt in this regard .Kindly supply the relevant rules.
8. If any body gets hurt or looses his life due to open main hole, is department responsible for it. Kindly provide the relevant rules.
9. Kindly supply the copy of citizen charter.
10. Who are the responsible officers to maintain these or cheque the open main holes .If open main Holes are found in the city what departmental action can be taken against them.


I understand that under the act , I should be entitled to are supposed with in 30 days .But this being an application involving a question of life and liberty , you are supposed/requested to answer with 48 HOURS of the receipt of this application as per RTI Act 2005

Application Fees of Rs ……………… is being attached along with this application in shape of IPO,PO Number ………………..Dated……………………..

That any other fees for supplying documents will be deposited as per your legal direction if I get with in stipulated time s per Act.

2. Which authorities are responsible for speed breakers.
3. The copies of chart/map showing there proper location .
4. That measurement of these speed breakers.
5. As per Indian Road Congress or as per other rules the measurement of the speed breakers,(height, breadth, marking on the speed breaker, sign board etc) what are the rules .Supply the copy of rules.
6. Whether unscientifically built road humps/speed breakers cause any harm to man and property if yes in what way kindly supply copy of relevant rules.
7. Number of unauthorized speed breakers in the city.
8. Who are the responsible officer for maintenance of road humps/speed breakers and removing of unauthorisedely built speed breaker.
9. What action can be taken against the officer in whose jurisdiction unmaintained, unauthorized speed breaker are found.
10. Latest survey report of the responsible officer, and report of the head of the department , regarding maintenance of speed breakers.
11. Also supply the details of unauthorized road cuts in the city ,under which rules these have been sanctioned .
12. If there are unscientifically and unauthorisedley built speed breaker till which date these will be removed.
13. What departmental action can be taken against the responsible if any point of time it is found that they have not discharged their official duty which they are bound to do as per rules.
14. Kindly supply the copy of citizen charter of the department.
15. The information is need in public interest.
