Model format for CPA Notice and case

Dear Sir,
I have filed an appeal against the PIO for not supplying information for more than 45 days, but I didn't get answer from the appellet authority also even after 35 days, wish to file in Consumer Forum, but I don't know how to file the same.
Kindly help me inthis regard. Pls provide me any model along with relevent sections to be used in consumer protection act.

This is the continuation of my earlier post. I request the senior members and activities to provide some of the materials in pdf form, so that it may be accessed by junior members like me to enlighten. If the materials made available like how our cases under R.T.I. Act will be maintainable before consumer court and how to file the case before Fourm? What are the relevent sections to be used for filing such case before the forum. Your informations will be more useful to me and all other juniors. Thanks and regards.
