Tracking PF Transfer Status

I had applied for transfer of my PF balance from SRO, Bommasandra, Karanataka on 30 May 2011. They had fwd the claim to RO, Kandivali, Mumbai office on 27-Jul-2011 for verification (obtained this information from EPFO GMS). Again used GMS portal to follow-up with Kandivali office and they replied me that the transfer application was rejected and returned on 20/Mar/2012. I had asked my current and previous employer about the returned/rejected application and both had indicated that neither had received the same. Then I had also followed up with SRO, Bommasandra and they had indicated that they had not received any rejected claim (as on 4th May). I followed up with Kandivali RO twice using GMS, asking specifi questions such as
a) Please tell me the name and address where you had sent the rejected claim/application (Form 13)
b) Please let me know the the reason for rejection
c) Please let me know the corrective action that I need to take

However, in response, all I got in a one liner saying "SIR YOUR F 13 IS REJECTED ON DT:20-03-2012.REASON IS SEND TO VERIFICATION.". At this point, I am unsure as to how to get the relevant information - I thought that perhaps I can use RTI here. How should I frame the query - should I give the similar background and ask the questions. Will it be prudent to ask for reason for delay in rejecting the claim?
