Can an RTI ask for individual's name? Something like this: 'Who was the:
1). Booking clerk,
2). Supervisor
at xxxx counter, yyyy place on zzzz date"?

1. Can this be extended to all PAs?
2. Can it be done to all levels in the official hierarchy of various PAs?

The Public Authority may go as per RTI Act and supply you information as may be available with them, irrespective of the level and hierarchy etc. PIOs are designated at all levels.
