Need Help: RTI application to get all the complaints ariased on him and notices sent him etc

An Bank employee with Designation Cash Peon has involved in Bank Fraud Cases(used to take the amount from customers and showed less amount ).The same employee has involved in my complaint(cash shortages) and the Investigation Team has dismissed from the Services.
The bank is not paying any Pension , the Retirement benefits to him.
I have already applied one RTI application and simply saying the employee has taken Voluntary , but the support documents are not providing to me.
Please help me writing the RTI application to get all the complaints ariased on him and notices sent him etc on record statements.

I am furnishing below, a model format from one of my earlier RTIs. You can amend / improve on it to suit your requirements.

1.Total number of reports /complaints/ allegations of / mis-appropriation of cash received from the public/ customers/ staff, against ...........................................................................during his career between 01.01.2000 to 31.12.2011, in the course of discharge of his functions as a public servant.

2.How many of these complaints have been attended to and investigated by the “competent authority”, in the Zonal Office or Head Office of the bank.

3.Please furnish me the details of guidelines to be adhered to and the procedure to be followed, whenever any complaint is received against an official of your Bank in the discharge of his/her functions as a public servant.

4.Has the Zonal Office or the Head office of the bank conducted any enquiry against the said cash peon, on the basis of the complaints/allegations?

5. If yes, please provide me the Name, Designation, Office Address, Office Telephone number of the Officer or Officers of the Bank who conducted such an enquiry.

6. Please provide me the Name, Designation, Office Address, Office telephone number of the Officer to whom the above mentioned enquiry report was submitted.

7. Please furnish me the details of all the actions taken against the said cash peon of .......... branch on the basis of such enquiry reports.

8. Please give me certified copies of all complaints / allegations of corruption/ harassment/ abuse of official power, as mentioned in Item 1 above.

9. I would like to inspect all files, records, file notings, information regarding the complaints etc. mentioned in Item 1 above, along with my friend/associate. During the inspection, I would like to take digital pictures of some of the records that I inspect. Kindly fix a suitable date and inform me of the same in advance to enable me to plan my visit to your office.

I am willing to pay any additional fees as demanded by the PIO, provided he gives me a reasoned order detailing the calculations as to how he arrived at the additional amount.

Please find enclosed DD No........... dated .............. for Rs. 10.00 drawn on ............., favouring ............ Bank A/c—Public Information Officer, towards application fee for this application.
