Framing RTI: RTI Application to CVC on Status

I have raised a complaint to CVC India on AndhraBank FatimaNagar and received Acknowledgement Copy in month of October 2011. After 3 months of time i have sent several Reminders for the Status of the complaint to CVO AndhraBank , CVC but there is not update on this.
Please help me in writting RTI Application on the Status , Actions taken. One bank employee is dismissed on this Matter. How to get these details also using RTI

Kindly refer to my Letter dated _____________________ vide which I have made a complaint (copy of the same is enclosed herewith) in this regard in capacity of being a citizen of India within the meaning of Section 3 of The Right to Information Act, 2005, following information is being sought under The Right to Information Act, 2005.

1. Please provide the certified copies of all the documents regarding the actions taken on our above said complaint i.e. all the information on record within the meaning of section 2 (f) read with 2 (j) of the RTI Act, 2005.

You are requested to abide by the provisions of Section 6 Clause 3 regarding the transfer of the said application. In case the requisite information or any part thereof is not available with your office please inform me regarding such transfer.

You are also requested to send all the correspondence/Information by speed post only, for which I’m ready to pay the charges if any. I’m enclosing a Postal Order No: ___________ for Rs.10/- (Ten) as application fee.

Sir, in case the correspondence/Information will be send by any other mode else than speed post & is not received by the undersigned then the entire responsibility will be yours. You are requested to provide the certified copies of the information.

Thanks sabharwal, how to get the dismiss copy of the Bank Employee in this case.., Bank is not paying any pension etc.
