Annual Statement Of Account for Provident Fund & Leave Wages

Hi,
Am Shivendra Vikram Singh, working in Action Construction Euipment Ltd. Ballabhgarh(Faridabad) posted in Jamshedpur(Jharkhand). Since from last two years am not getting my annual statement of Provident Fund and my pay slip too. Can any one help me out to know how to get it on line, so that could know the status. Just also wana know that if any leave is not taken from the company through out the job time then does the company pay back to its employee in every end of financial year or it pays back only when the employee leaves the company.
