How to apply if format not prescribed in state rti rules

Format of RTI Application for Information of State Govts, where format is not prescribed under State RTI Rules

Some states have prescribed format for RTI application under their RTI rules, which needs to be used for that state. Many states have not prescribed such a format. In such cases attached format can be used.

I applied for passport on 28th jan,2008 vide file no cal/A/007910/08 but I neither got the passport, nor I get information for non issuance.

Even if there is no format, sample application from this portal may be used by changing the required information. I have used such application very successfully.

Each city has different Registrar of Coop Societies, You may google search.
