filling a RTI application

I have few issues
1. I have drafted an application, i don't know whether this is right or wrong ( I am attaching the application )
2. How do i pay the fee? should i right away pay the Rs 10 or should i ask the PIO to tell me the amt of fee which needs to be paid
Please guide me how to proceed further
