    Just address is to:

    CPIO under RTI Act 2005
    DoPT
    North Block
    New Delhi - 110011

They will decide who is the appropriate CPIO dealing with this matter.

Send the application by Speed Post, track it online and take a printout after it is delivered. that will be your proof of delivery / and date.

Moreover, the guidelines for reporting / reviewing officers are very much printed on the APAR form itself. You can also access the guidelines in DoPT website.

Most of the recent guidelines are available in this thread also: http://www.rtiindia.org/forum/118-disclosures-annual-confidential-reports-acr-comes-under-official-secret-act-37.html
